Team Members with Central Florida Health insurance: Use this directory to identify which physicians are classified as Tier 1 Providers. Using a Tier 1 Provider ensures that you will receive the highest level of insurance coverage for your visit and services.
On this list, you will notice that some providers and specialties are highlighted in red.
- Physician’s name or specialty in red: To maximize your insurance benefit coverage and have your claims for the office visit paid at the highest level (Tier 1), you must use a physician highlighted in red.
- If a specialist is not shown in red, it means you can choose a provider from this list or you can choose any provider, for this specialty, from the BCBS “BlueOptions” Network and receive coverage at the highest level (Tier 1) for the office visit. Click this link to find a participating physician in the “BlueOptions” Network.
- If the specialty you need is not on this list: You can select any provider for this specialty from the “BlueOptions” Network (click this link to find a participating specialist in the “BlueOptions” Network) and you will receive coverage for the office visit at the highest level (Tier 1).
Important: If your doctor wants to do a lab, diagnostic image, or other ancillary procedure or service in the office during your visit, remind him/her that in order to have your health claims paid at the highest level (and minimize your out-of-pocket expense) you will need to get those procedures or services done at a Central Florida Health facility when applicable.
To receive the highest level of coverage for any procedure (i.e. lab work, diagnostic imaging, etc.) or hospitalization/inpatient stay, you must use a Central Florida Health facility. If you need to have a procedure done elsewhere (including in any doctor's office), you must contact Health Advocate before you receive the service/procedure to complete the pre-approval process.
Contact Health Advocate and complete the "Request for Waiver to Tier 1 Pre-Approval Form" before receiving any service/procedure outside of a Central Florida Health facility. Failure to complete this form will result in coverage dropping to Tier 2 or Tier 3 levels and you will not be able to have your service/procedure waived to the Tier 1 level, making the out-of-pocket cost for you higher.
